From 1 July 2022, Clinical Commissioning Groups (CCGs) will be dissolved, and Integrated Care Boards (ICBs) will take over the responsibility for NHS functions and budgets. A new survey is asking parents of children and young people with disabilities for their views on Short Break services.

NHS Hull Clinical Commissioning Group (CCG) and Hull City Council are working with Hull Parents Forum to understand what activities children and young people can access to give them and their families a Short Break.

We want to understand what barriers there might be preventing access to these activities and how we can improve these services across the city.

This survey is anonymous and your response is important in helping to shape future services.
