Better care in Hull
Better Care in Hull is NHS Hull CCG and Hull City Council’s shared vision of integrated local health and social care services.
The Better Care programme was established in 2014 as part of a government initiative, the Better Care Fund. The key aims of Better Care is to:
- Offer care closer to home.
- Care provided by the right health and social care professional.
- Reduce the demand on A&E.
- Reduce hospital admissions.
- Keep people living independently as long as possible in their own home.
Our Better Care in Hull plan looks at how we can support older people better.
Better Care in Hull focuses on services for older people in the first instance. Older people in Hull have told us that it’s important to them to be able to stay in their own homes and remain independent for as long as possible; this will be at the centre of any service we provide.
We’ll look at how we can make it easier for older people to access health and social care support in their local communities whenever they need it. We have also developed the Hull FIRST Response Service which provides a fast response and practical support for people who have fallen.
In 2017 we are continuing to engage with people and develop our Better Care in Hull plan for 2017-2020.
The local vision described within the Hull Better Care plan represents our shared aspirations for Hull. Our plan outlines how will:
- People receive the right care at the right time.
- People receive care closer to home, as an alternative to hospital.
- People with complex needs have a named professional.
- Focus on individual needs rather than a service.
- Maximise on community assets (e.g. Social Prescribing).
Our 2016 / 17 plan has identified 4 areas of focus:
- Prevention
- Intervention
- Rehabilitation
- Rapid Community Response
We will continue our Better Care Plan for 2017-2020.

NHS Hull CCG and Hull City Council are continually developing and evaluating the Better Care in Hull plans. As we move in to 2017 and beyond we will be developing these plans, through consultation and engagement, with people across the city.

The progress of our Better Care in Hull programme is shared through a quarterly newsletter which is aimed at both professionals and the public. The newsletter is circulated to all GP practices in Hull, Hull City Council buildings, libraries and with health and social care staff.
