Hull City Council Public Health and NHS Hull CCG are re-procuring a Stop Smoking Service for the City.
To ensure the service is fit for purpose and meet the needs of its users and those who refer into them, we are undertaking consultation.
To enable us to gain the most comprehensive feedback we are asking ex-smokers and stakeholders to complete a brief online survey.
The survey is anonymous; you will not be contacted for further information or promotion.
Your experience and opinions will help shape a future service.
