Community cardiology services
Three community cardiology (heart) services are available in Hull to improve health outcomes through earlier diagnosis and treatment of common cardiology conditions.
The services are designed to be responsive to your needs and allow you to be seen quickly at a time and place convenient to you. You must be referred by your GP.
Community outpatients cardiology service provided by Concordia
Community Outpatients offers a consultant led cardiology service in Hull. The service reduces waiting times for patients requiring non-invasive cardiology services and diagnostic tests. Patients are seen at a location convenient to them within 4 weeks of the service receiving their referral. The outpatient service is run by specialist staff and is offered at:
- Wolseley Medical Centre, Londesborough St, Hull, HU3 1DS
- Newland Group Practice, 61 Alexandra Road, Hull, HU5 2NT
- Wilberforce Health Centre, 6-10 Story Street, Hull, HU1 3SA
Each clinic has a large number of appointments across various days to provide the greatest flexibility available for patients.
You must be referred by your GP in one of the following ways:
Post: Direct referral can be made via post. To use the pre-paid service simply write ‘FREEPOST COMMUNITY OUTPATIENTS’ on an envelope and place it in a postbox. No stamp is required.

Community cardiology service provided by City Health Care Partnership (CHCP)
CHCP Community Cardiology Service introduced a ‘one-stop’ service for patients 18 years and older registered with a Hull GP. The service offers quick and easy access to cardiology diagnostics (ECG, 24hr ECG, 24hr ambulatory blood pressure monitoring, home based blood pressure monitoring and echocardiography) and consultant consultation.
The Community Cardiology service will complement the Cardiac Rehabilitation, Arrhythmia and Heart Failure services delivered by the Specialist Nursing Teams.
